Claim forms are mailed directly to the listed owner unless the owner is deceased. If the listed owner is a company or other organization, the claim forms will be mailed to the company address to the attention of an officer or other designated employee.

The claim form will provide more specific information regarding this account such as the amount, what bank or company sent the money to Georgia and the last known address of the owner.
Receipt of the claim form does not guarantee payment of the claim. You must provide the documentation requested on the claim form to prove your ownership of the property.
